Supporting documentation
Please make sure that you provide the correct supporting
documentation with your application form:
Supporting documentation: Singapore residents / permanent
residents
- Proof of ID: For walk-in, please bring your
original Identity Card to City Index Asia Pte Ltd. You may send in
a copy of your ID Card by post to City Index Asia Pte Ltd. Please
do not send originals of documents as they will not be
accepted.
- Proof of address: In the form of a recent bank statement,
utility bill or telephone bill. Please note that we do not accept
mobile phone bill as proof of address.
- For both FX and CFD applications: A printed
copy of our applicable Risk Disclosure Statement signed by you
and a witness.
Supporting documentation: Non Singapore citizens / non
permanent residents
- Proof ID: For walk-in, please bring your original Identity Card
and/or Passport to City Index Asia Pte Ltd. You may send in a copy
of your ID Card and/or Passport by post to City Index Asia Pte Ltd.
Please do not send originals of documents as they will not be
accepted.
- Proof of address: In the form of a recent bank statement,
utility bill or telephone bill. Please note that we do not accept

- For both FX and CFD applications: A printed
copy of our applicable Risk Disclosure Statement signed by

Supporting documentation: Corporate
- Certificate of Incorporation / Certificate of Partnership /
Certificate of Registration
- Memorandum and Articles of Association
- Certified Extract of Directors’ Resolution
- Company Ownership Structure
- List of Directors and Shareholders
- Business Profile from ACRA dated not more than 1 month before
account application (for company incorporated in Singapore)
- Power of Attorney for person(s) authorized to act on the
Company’s behalf but is/are not named under the section
"Particulars of Persons Authorised to Deal on Behalf of the
Company"
- A printed copy of our applicable Risk Disclosure Statement
signed by the authorised signatories and a witness, and
- Any other documents which may be requested by City Index Asia
on a case to case basis.
Note:
- For companies incorporated in Singapore, a company
Director and Secretary must certify that all documents
submitted are true copies, with the company’s stamp affixed.
- For companies not incorporated in Singapore, please arrange for
a Notary Public or Commissioner of Oaths to notarize the signatures
on the account application and all attached documents.
